HITECH Acronyms that every Doctor's Office should learn - November 30, 2009

Orlando Florida -- Thanks in part to the work of Sencilo HealthIT Solution for effort to create an NHIN and implement electronic medical records/electronic health records (EMR/EHRs) available through RHIOs while still complying with HIPAA has created an alphabet soup of terms, acronyms and initials in various combinations.

To help you sort out what all these terms mean, we have created a short glossary. This is Part 1 (Part 2 will be published tomorrow). Because almost everything here seems to be subject to change, this will be a living document of sorts.

AHIC – American Health Information Community

The AHIC was a federal advisory panel created by HHS to make recommendations about the implementation of health information technology. In November 2008, after making more than 200 recommendations, it was decommissioned and became a private organization called the National eHealth Collaborative.

ARRA – American Recovery and Reinvestment Act (ARRA)

The ARRA is a law approved by the 111th congress and signed into law by President Obama in February 2009. Also called the economic stimulus bill, ARRA authorized the spending of more than $750 billion to kick-start the U.S. economy. Part of the ARRA is the HITECH Act, which authorized more than $20 billion in incentives to help the health care industry adopt health information technology and EMR / EHRs.

ASTM

ASTM International (formerly the American Society for Testing and Materials) is an international standard-setting organization based in Pennsylvania. Over more than 100 years, it has created thousands of standards in industries as diverse as plastics, rubber and metals.

CCD – Continuity of Care Document

CCD is a set of specifications and standards for electronic medical records that was developed by HL7. The CCD specification takes HL7s Clinical Document Architecture and combines it with elements of ASTM’s CCR specification.

CCD is one of two primary standards for the electronic assembly and transmission of medical records. The other is CCR. So far, neither proposed standard has been adopted by CCHIT for use in EMR/EHRs. Chances are you won’t even know which standard a program uses — the rules mostly operate at the programming level. For the user entering and sending data, the whole thing will be pretty much invisible.

CCR – Continuity of Care Record

CCR is a set of specifications and standards for electronic medical records jointly designed by the ASTM, medical professionals and health informatics specialists. The continuity of care record contains the core information needed by doctors to treat their patients: patient information, insurance data, medications, diagnoses and problem lists, allergies, etc. The CCR is designed to be easily created by a provider after a patient encounter. The CCR uses the XML data interchange language, so it can easily be read, translated, understood and transmitted by EMR/EHR software.

CCR is one of two primary standards for the electronic assembly and transmission of medical records. The other is CCD. So far, neither proposed standard has been adopted by CCHIT for use in EMR/EHRs. Chances are you won’t even know which standard a program uses — the rules mostly operate at the programming level. Again, for the user entering and sending data, the whole thing will be pretty much invisible.

CCHIT – Certification Committee for Health Information Technology

The CCHIT is a non-profit organization founded in 2004 for the purpose of advancing the adoption of Electronic Health Records and health information technology. CCHIT has been designated by the U.S. Government as the organization responsible for certifying EMR/EHR software. If you want the incentives available from the government to adopt HIT, you’ll have to use CCHIT-certified products.

CDR – Clinical Data Repository

The clinical data repository is a name for the database where all the information about a patient resides. It includes such information as X-rays, blood test results, medications and care notes – a continuous, ongoing record of a patient’s medical history.

CMS – Centers for Medicare and Medicaid Services

The CMS is the blanket government organization that manages both the Medicare and Medicaid programs. It is also the organization responsible for disbursing the $20 billion in incentives available for the adoption of HIT.

CPOE – Computerized Physician Order Entry

Every time a provider wants an action to take place, such as giving a patient a medicine, ordering blood samples or changing a diet, he or she writes an order. In the past, the doctor wrote the order by hand. but paper orders can be lost or improperly transcribed, leading to potential problems. With a CPOE, the doctor writes orders using a computer. Patient data and decision support tools are right there to help. One great benefit of CPOE – especially when it come to medicines – is that it eliminates any problems related to poor handwriting.

Decision Support

Decision support systems (DSS) are computerized programs that can help doctors make decisions. A DSS for prescribing, for example, will generally include a knowledge base of drug interactions. A doctor puts into the program a list of a patient’s current medicines and a diagnosis. The decision support system compares that list to its knowledge base and gives the doctor a list of medicines he could prescribe that won’t create an adverse interaction. The DSS helps the doctor and saves him time, but it does not make the decision in the doctor’s place.

EHR/EMR/PHR– Electronic Health Record/Electronic Health Record/Personal Health Record

We put these three together because people often use them interchangeably. “EMR” stands for Electronic Medical Record, “EHR” stands for Electronic Health Record and “PHR” stands for Personal Health Record. There seems to be a great deal of controversy over which term means what. The definition seems to vary depending on who is doing the defining – a vendor, a doctor or a consultant. Well, to add to the confusion, here is what the government says. The following three definitions were taken from the U.S. Department of Health and Human Services website.

An Electronic Medical Record is “An electronic record of health-related information on an individual that can be created, gathered, managed, and consulted by authorized clinicians and staff within one health care organization.”

An Electronic Health Record is “An electronic record of health-related information on an individual that conforms to nationally recognized interoperability standards and that can be created, managed, and consulted by authorized clinicians and staff across more than one health care organization.”

A Personal Health Record is “An electronic record of health-related information on an individual that conforms to nationally recognized interoperability standards and that can be drawn from multiple sources while being managed, shared, and controlled by the individual.

Make of these what you will, because chances are the working definitions will change even more as time goes on.

Encryption

Encryption is the use of a mathematical algorithm (called a cipher) to take unencrypted data (plaintext) and make it an unreadable jumble of characters (ciphertext) to anyone who doesn’t have the unique tool to unencrypt it (the key). To anyone intercepting an encrypted message, it will look like gibberish. The key is not a physical key, but rather a string of characters (letters, numbers and symbols) that will tell a computer how to convert the data back into plaintext.

HCPCS – Health Care Common Procedure Coding System

This is a numbering system used by anyone who bills Medicare. Every product or service Medicare might pay for has an alphanumeric code to be used for billing purposes. In order to be reimbursed by the government, the doctor must use this billing code system.

There are two primary levels of codes – Level I and Level II. Level I codes are identical to the CPT codes created by the American Medical Association, and include services rendered by doctors, such as a flu shot (code 90658) or stitching up a 1” cut (code 12002). Level II codes will cover the gauze bandage (code A6216) or the wipes to clean the wound (A4245).

HHS – Health and Humans Services

Health and Human Services (HHS) is the department of the U.S. government that is responsible for overseeing the implementation of EMR/EHRs under the HITECH Act. Medicare and Medicaid are part of HHS.

Health Informatics

Health Informatics is the use of computers and information management in a health care setting. Things that fall under the banner of health informatics include EMRs, medical coding systems, practice management software and medical records management.

Speech Recognition and EHR software - Enabling Medical Office Efficiencies - November 29, 2009

Orlando Florida -- In the beginning there was your memory. The physician's memory was the original repository of the patient health record. Memory was supplemented by handwritten notes on papyri in ancient Egypt and Babylon and on paper from medieval times to the 20th century. With the advent of recording devices in the 20th century, handwritten notes gave way to the infinitely more time effective practice of dictating patient notes into a recorder which were then transcribed into a typewritten or word processed document. Of course, that practice introduced an extraneous third party into the medical record keeping process: the transcriptionist with the attendant additional expense to the physician and loss of privacy for the patient.

In the 1990's, speech recognition software came to market and products such as Dragon NaturallySpeaking® enabled physicians to either speak directly to their computers to produce patient records and reports instantaneously or to continue dictating on recorders which could then be ported onto a computer so that a digital voice file could be transcribed by the software to produce the patient record. The transcriptionist was often eliminated from the process and efficiency increased in terms of time and money spent to produce the patient record, says Brian McCarthy CEO and founder of Sencilo HealthIT Solutions and well know guest speaker on voice to text technology.

At the current time, 21st Century technology offers physicians and health care providers a medical record paradigm that will not only vastly upgrade the process of producing, maintaining and safeguarding medical records but will, in a direct and fundamental way, actually improve the quality of medical care. The technology is Electronic Health Records (EHRs). EMRs produce the most accurate and complete patient health record possible to date and help physicians practice better medicine as well. EHRtechnology is available in a plethora of shapes and sizes with a great variety of possible features. The technology can change the way you interact with your patients, from before they make their first appointment to after they've left your office, and have questions about their visit in your office, says McCarthy.

As a practicing physician you are aware of the repetitive nature of some aspects of your practice, specifically with regard to patient diagnosis. It is very likely that you and/or your staff have asked the same or at least very similar questions to each of the thousands of patients you have treated. Unless you are practicing in a tertiary referral center, and never see the same condition twice, the patient answers likewise tend to be repetitive. Similarly, physical examination findings fit into certain categories that are seen over and over again. For this reason, most of the current high-end Electronic Medical Record products very capably utilize ‘pick lists' or ‘click and point' methodology to complete large portions of the patient medical record.

These point and click systems are particularly adept at documenting, for instance, allergies to medications, medications that are currently being taken, past medical history, family history, social history, and major portions of the physical exam. This is the case because of the narrow range of options which are available as patient responses. For instance, your patient either smokes or doesn't smoke. And if he/she smokes, it is probably 1 ppd, or 2 ppd, or some other value that can reasonably easily be foreseen by the experts who have designed the point and click system for your office.

However, the historical portion of the patient medical record typically has a great deal of information that cannot be easily foreseen by the developers of the point and click templates. For instance, as an Orthopedic Surgeon, my patients frequently find themselves in automobile accidents. It is not likely that the author of whichever EHRmay find its way into my office has contemplated the various street names and intersections in my community. Therefore, in a typical point and click system, there will be a scarcity of relevant information concerning the specifics of the accident. And I find that these specifics are important for a wide variety of reasons, not least of which is that they remind me of the particulars regarding this patient when they return to the office. Utilizing templates for the historical portion of the report, while feasible, tends to produce extraordinarily repetitive reports, each of which sounds not only vaguely similar to the previous patients, but in many cases essentially identical to other patients. This certainly makes it difficult to recall the characteristics of this particular patient.

Based on extensive, personal experience I can unequivocally state that production of a medical record with a system that restricts the user to point and click templates exclusively is far from perfect. At one point in time I undertook the academic exercise of trying to put together a decision tree regarding the mechanism of injury for the occupant of a motor vehicle who was involved in an accident. I started with a Microsoft Access database and started branching. First, was my patient the driver, front passenger, or rear passenger? For each of these 3 options, I then wanted to know if there were 1 car, 2 cars, or multiple cars involved. We now have 9 options. Then we need to know if the car was stopped, traveling slowly, traveling at a moderate rate of speed, traveling quickly, or traveling at an unknown rate of speed. With these 5 options we are now up to 45 mechanisms of injury. Was a seat belt in use; was the headrest properly positioned. 4 more options, and now we are at 180 choices. Was the vehicle impacted from the front, rear, left side, right side, left front, right front, left rear, right rear. Another 8 options to consider. Next, did any portion of the patient impact any portion of the vehicle? This gets a little more difficult to enumerate, but I classified the body into 8 different areas that were likely to have been injured. Then I wanted to know was there a substantial amount of damage to the vehicle, etc.

Eventually, I had an untold number of different mechanisms of injury for an automobile accident, each of which could very easily be documented for each of my patients. It would merely take a few clicks of a mouse, and I immediately had paragraphs of text on my document which covered, or so I thought, essentially every possible option.

Did it work? My answer is a resounding NO. This was, as I mentioned, a ‘home grown document management system', and I could customize it at will within my office. I found that in at least 50% of the patient visits, I needed to change the templates since I had not, in fact, taken into consideration every possible mechanism of injury.

So, what did I do? Like any physician who knows that he can make the system work, I modified the templates each and every time that I found it necessary to do so. I figured that after seeing ‘enough' patients, the percentage of new patients who didn't fit into the predetermined potential mechanisms of injury would diminish, and when it was below 10%, I'd stop customizing it. One flaw persisted; the number of new patients who didn't fit into the mold never went below 50%. It just stayed there. No matter how many different options I placed into the template, my patients just hadn't reviewed my EHRbefore having their accidents, and therefore hadn't complied with my predetermined mechanisms of injury!

I either needed to choose a different patient population – one that was more considerate of the templates that I was using – or I needed to discontinue the use of the templates. I didn't like either option, and I quickly determined that the middle ground was best. I kept my patients, and I kept my templates. However, I utilized the templates only for the portions of the reports for which they are most appropriate. I need to test the range of motion of the spine on all of my patients who complain of spinal pain. The AMA published a book which tells me what the ‘Normal Values' are. I made templates out of the MS Word macros I had been using for years, and in just a few short clicks, the entire range of motion was documented.

But what did I do for the pesky history that was so difficult to document properly? I resorted to the state of the art of the 1990's and implemented Dragon NaturallySpeaking speech recognition software. I dictated the history using Dragon, and used templates for the rest of the medical report. It worked perfectly.

Therefore, I recommend to any physician that he or she integrate Dragon NaturallySpeaking speech recognition software into their EHRsystem to produce the best and most complete patient health record. This is because the pre-determined templates of most EMRs simply cannot anticipate the full spectrum of facts presented by the patient which must be incorporated into the historical portion of their health record. The historical portion of the patient medical record should not be truncated to fit a narrow range of template options and speech recognition technology enables the physician to dictate the entire patient history and any other patient data into the EMR.

One of the advantages of an EHRis that it allows physicians, hospitals, insurance companies, pharmaceutical companies, medical societies, and other parties entitled to view the patient data for legitimate, permissible purposes, to do so. Legitimate, permissible purposes include coordinating patient treatment, accessing diagnostic procedures and results, preventing adverse drug reactions, and ensuring medical practice within clinical practice guidelines. One particularly high priority purpose from the physician's standpoint is that the data be accessed by third party payors to streamline reimbursement for services.

An issue that arises with the integration and use of speech recognition software in an EHRsystem is that the voice dictated text may not be maintained adequately as ‘data' and therefore may not be appropriately parsed with current technology. It is therefore not easily accessed by third parties, and may not be able to be utilized, for instance, for E/M coding. However, the dictated text is generally not information that third parties require. For example, in the context of the particular facts of an automobile accident, I do not believe that the E/M code will be different for a front-end, 1 car collision than it is for a 2 car rear-end collision. Similarly, it is highly unlikely that there is any difference in medical care for a patient who develops chest pain at their favorite local restaurant, as opposed to developing it while they were at the fair. Again, if the child's earache started after they swam in their backyard pool, as opposed to the local municipal swimming pool, the treatment provided is likely to be similar.

Even though the historical portion of the medical record which is dictated does not constitute ‘data' that can be captured by third parties, given the current state-of-the-art, this is a slight disadvantage with no substantive impact. Any perceived disadvantage is significantly outweighed by the benefits to the treating physician and the patient of having a complete patient history on file which the physician can access and utilize in rendering proper medical care.

There is a trend towards utilizing artificial intelligence to parse the dictated historical portion of the record so that the EHRcan capture it as data. However, I do not believe that there are any reasonably priced, commercially available products which do this with sufficient accuracy at this point in time as to be practical. However, in the not too distant future, this is likely the direction that dictated text will take.

One of the limitations of speech recognition technology in the past was that the level of accuracy only reached 96 or so percent. However, the latest versions of the software, combined with modern hardware configurations, has resulted in a 99% accuracy rate except for those physicians who speak with particularly unusual and heavy accents. It is generally accepted that a 98% accuracy rate for a human medical transcriptionist is the standard. Most physicians should be able to obtain at least 98% accuracy without any significant difficulty, and 99% accuracy is more and more common using the current version of Dragon NaturallySpeaking Medical. In fact, I have seen published reports of physicians with accuracy rates of over 99.5%.

Another factor which may concern a physician is the need to wear a headset microphone which is physically attached to a computer. While this is the ‘standard' mechanism of utilizing voice recognition software, there are a variety of options. One is the use of a wireless headset, or even an array microphone which can be attached to the computer, but not to the physician. A more and more common alternative is the use of a high-end digital recorder, such as those by SONY®, Philips®, or Olympus® to capture the spoken voice, which can then be ported to a computer that then analyzes and transcribes the dictated text. In situations such as this, the medical office will frequently utilize the services of a ‘revisionist', to review the text, as opposed to a transcriptionist who classically types it from scratch.

In step with hardware that permits computing in a mobile environment by means of a tablet PC or laptop, the newest release of Dragon NaturallySpeaking includes a roaming user profile. This allows physicians to dictate, using their own personalized user profile, from any of a variety of networked computers, PCs, tablets, etc.

The need to integrate speech recognition technology into EHRs has not escaped the notice of the EHR manufacturers. It is no longer the exception but has become one of the standards by which EHR products are evaluated. Does the EHR have a speech recognition feature? Is it compatible with Dragon NaturallySpeaking?

Allscripts EHR Solutions are Guaranteed to meet CCHIT Requirements - November 24, 2009

Web based EMR - Pros and Cons - November 11, 2009

Orlando Florida --- Times are tough economically and it may seem like a bad idea to spend money on implementing an Electronic Medical Records solution in your practice. However, a large number of physicians continue to be amazed at the increase in efficiency of their operations after successfully implementing the ‘right-fit' EMR in their Clinics.
Web-based EMRs have been touted as a nice way to implement EMR solution with low initial investment. Web-based and client/server EMR systems have unique advantages and disadvantages. Which model you should choose for your practice will depend on your special requirements.

Despite the obvious advantages of web-based EMR (access from anywhere, remote hosting, etc.), there are some important points you should consider when deciding whether to implement a web-based system:

1. Don't imagine that a "high-speed" Internet connection is all you need to run a Web-based EMR. It's important to find out if you'll actually have the bandwidth required to send, receive, and exchange data efficiently. Consult the EMR vendor about its requirements, and talk to your Internet service provider about its available bandwidth options.

How much bandwidth you need will depend on the number of users and the volume of data flowing between your practice and the hosted Web- based EMR. If the data increases, you may need additional bandwidth, and you'll need to be prepared. Carefully review the EMR vendor's recommendations before you sign up for a bandwidth plan.

2. How reliable is your Internet connection? This is critical - and also, your upload and download speeds may vary depending on the time of day and how much bandwidth is being used by nearby businesses. You'll want to plan for extra bandwidth to avoid slow-downs in charting your patient encounters.

If you share an Internet pipe with other businesses in the same complex, your data transfer speeds may fluctuate dramatically, depending on how the others businesses are using the connection. We recommend asking your Internet service provider for documented evidence of the uptime you can expect. Depending on the answers, you may want to consider getting a dedicated pipe for your clinic. Work with the EMR provider to develop a backup system for charting on paper if the system goes down, and develop your office processes accordingly

3. What if your EMR vendor folds its tent? Will you be able to retrieve your data from the vendor's servers? Will the data be in a format that you can use/convert/transfer?

After deciding on a web-based EMR vendor, treat the vendor as your partner. You should be able to discuss every possible scenario with the vendor's sales reps. Better yet, work with an experienced EMR consulting company who'll help you communicate with the EMR vendor. Consider asking the vendor to provide data-backup files on a regular schedule.

4. Unlike a client/server environment, where you have local control over the server and EMR software, a web-based EMR makes you dependent on the vendor's customer support. Will your vendor reliably and promptly support you and your staff by phone, IM, email, or chat? Will the vendor be available to quickly help you solve urgent or critical issues? Talk to the vendor about its support structure, especially outside normal working hours.

5. What will happen if you someday decide to move from a web-based to a server-based EMR? Does your vendor offer that flexibility? If so, how seamless will the migration be? How much will it cost? Even if you don't believe it will ever happen, it's in your best interest to discuss it with the vendor today, and get an in-depth understanding of the process.

We've seen many successful web-based EMR systems. They do work well for most physicians. By understanding the handful of caveats we've outlined above, and preparing accordingly, you'll stand a better chance of implementing a successful web-based EMR system for your practice.

How to get $64,000 Medicaid Incentives for implementing Certified EMR / EHR Software - November 11, 2009

Orlando Florida --- Medicaid Incentives are available for physicians, hospitals, nurse practitioners, nurse mid-wives, pediatricians, rural health clinics, and Federally Qualified Health Centers who are not hospital-based and have Medicaid patient volumes as follows:

For physicians, nurse practitioners, and nurse midwives in private practice, the Medicaid threshold is 30 percent of patients. Pediatricians seeing more than 20 percent Medicaid patients but less than the 30 percent of the threshold could receive a reduced incentive payment.

For physicians, nurse practitioners, nurse midwives, and physician assistants working in either rural health clinics or federally qualified health centers, the threshold is also 30 percent; however, they can include SCHIP enrollees, patients for whom no payment is made to the clinic for their care, or patients to whom a sliding fee scale is applied.
Medicaid Incentive Payments

Presently, the Act does not specify a start date for Medicaid incentives however, leading industry experts believe it will be in parallel with the Medicare Incentive timeline or as soon as 2011.

Medicaid Incentive payments are set up to help offset some or all of the actual initial costs of a Certified EHR system plus, additional annual payments for support.

In year one, this provider group is eligible for a payment amount of up to 85% of the net allowable cost for their Certified EHR technology, and is paid up to a maximum incentive payment cap of $25,000. This year 1 payment cannot include any support or related costs expected during the following four years after initial purchase. However, to offset future support only costs, each provider can earn up to $10,000 annually, for four additional consecutive years to support the “meaningful use” of electronic health records. (Medicaid Pediatricians are eligible for 2/3 of the amount otherwise specified.)

Best Practices in selecting EMR Software - November 11, 2009

Orlando Florida --- When you take on the onerous task of selecting the right EMR Software (aka EHR Software) for your clinic yourself, you may unknowingly do many little things that will surely lead to selection of EMR Software that is not right for you. They will result in costly and frustrating mistakes that can come as a surprise and hit you hard when you least expect them, says Brian McCarthy CEO of Sencilo HealthIT Solutions and well-known speaker of the subject of IT for Healthcare providers.

1. Not taking the staff into confidence before starting the EMR selection process. Your staff will be most impacted by the changes in your clinical environment. Hence it is imperative that you help remove the fear of the unknown from the mind of your staff by explaining how implementation of the selected EHR Software will make them more efficient and effective in their day to day operations.

2. Not drawing up the criteria for your decision making process that is objective with a measurable metric rather than basing it on gut feeling. Criteria should be appropriately grouped and weighted so that you can arrive at a weighted score for each EMR being evaluated. If you are evaluating three or four EMRs and do not have the criteria well defined, chances are huge that some irrelevant issues will cloud your vision and impact your decision making.

3. Not following through with the references or not doing a thorough job with the references. You may often believe that all references are going to speak highly about the EMR since they have been handpicked by the vendors. However, as we all know, physicians are a closely knit community and it is not difficult for you to validate or repudiate what the references told you about their experience with the EMR Software from companies like Allscripts and Vendors like Sencilo HealthIT Solutions.

4. Accepting at face value what EMR vendors tell you about all their EMR Software features and functionality. For example, we have found some of the key differentiators claimed by some vendors had actually been decommissioned because they did not work as intended and were back to the drawing board for a re-work.

5. Assuming that larger and well known EMR vendors will be technology-wise more advanced, and customer-support-wise friendlier to your individual needs and situation. This assumption could be quite misplaced because bigger is not always better as many of these vendors are too busy working on tying up bigger alliances with hospitals and IPAs trying to rope in hundreds of providers in one scoop. Thus, ramping up customer support for small practices almost always takes a back seat and therefore you may be left out in the cold when you need help right away. Larger vendors are less likely to hear you out in regards to your individual needs and even more importantly, make changes to the software to meet your genuine requirements.

6. Selecting a vendor who says that their EMR Software does not need much training and it is easy to pick up. If you and your staff do not get adequate training, you will not be able to utilize the software to its full extent. There are no plug and play systems that can be implemented in a few minutes, few days, or even few weeks. Negligible amount of training built as part of initial implementation proposal is nothing more than a contrived method to win your business.

7. Being too focused on price early-on in your search or believing that there really is such a thing as Free EMR Software rather than focusing first on what you need for optimizing your clinical processes and workflow. A lowest cost approach does not typically translate into saving a lot of money and a lower Total Cost of Ownership. Once you determine what you want and need, you may start drilling down into pricing the EMR Software alternatives you are considering. Don't get pushed into making a decision, be patient.

8. Selecting an EMR on the basis of current requirements alone and not considering your foreseeable or possible future needs. Growth is generally inevitable and you may take on new providers (multi-specialty?), offer new services, or use devices that need to interface with the EMR.

9. Believing that you or an appointed member of your practice has all the time, knowledge, and experience to handle almost a monumental responsibility for undertaking the research, screening, selection, negotiating, and implementation of the best suited EMR Software for your practice without guidance from an experienced EMR consultant. There are too many decisions to be made and if you stop and think about it, you may realize that you truly do not know all about the good, the bad and the ugly, about EMR selection, implementation, price negotiation, hardware selection and more. Engage with an EMR Consulting company to assist and work with you.

10. Not having a true picture of how the EMR will help create a true paperless office and what components will or will not be addressed by the vendor directly as part of implementation. You must ask the EMR vendor to adequately demonstrate that their EMR Software will in fact increase efficiency and translate into increased revenues.

11. Not taking enough time to discover and evaluate the initial and recurring costs of implementing EMR. This also includes understanding the technology underpinnings required to support the selected EMR. It would also greatly help if you define and require the line items expected on the proposal from every EMR vendor you are considering so that you can make the best apples to apples comparison between different EMR Software.

12. Not clearly understanding what the vendors always mean by that or this is included at no additional charge. A good example of this is not clearly defining what they mean by a bi-directional interface with labs. There are actually two costs involved in setting this up and usually EMR Vendors mean that their side of the cost is included and that they can accommodate the connection but, someone (you) will have to pay for the other side of the connection.

Walgreens and CVS see 185% increase in Electronic Prescriptions over 2008 - November 10, 2009

Orlando Florida -- Walgreens officials say the national pharmacy chain's electronic prescriptions reached the 4 million mark in October - a 185-percent increase over October, 2008.

According to officials, the milestone demonstrates the impact of the Medicare Improvements for Patients and Providers Act of 2008, which provides financial incentives for practitioners who use electronic prescribing in 2009.

Walgreens will fill more than 45 million electronic prescriptions in 2009, compared with 15 million filled in 2008, officials said. The company expects growth to continue with help from financial incentives in the federal stimulus package, which encourages hospitals, doctors and others to adopt electronic health records, a core component of which is e-prescribing, says Brian McCarthy CEO of Sencilo HealthIT Solutions.

"With the federal stimulus package providing $19 billion in incentives to adopt electronic health records, doctors will gain easier access to software that makes electronic prescribing possible," said Don Huonker, Walgreens' senior vice president of healthcare innovation.

Walgreens officials are pushing for e-prescribing to be extended to all prescriptions, including controlled substances. Currently, the U.S. Drug Enforcement Agency does not allow doctors to electronically prescribe controlled substances, which account for approximately 15 percent of all medications prescribed. As more medical office realize cost savings of their own, says Mccarthy.

"Continued growth in e-prescribing will help meet the goals of healthcare reform," said Huonker. "In addition to lowering costs for doctors and pharmacies, electronic prescribing makes it more likely that patients will get their prescriptions filled, benefit from their drug therapy and avoid more expensive medical procedures. E-prescribing also reduces the medication errors and adverse drug events that cost the healthcare system billions of dollars per year. And by eliminating the need to call a doctor's office to verify a prescription, e-prescribing provides pharmacists more time to focus on drug interactions, the right dosing and patient consultation."

Ambulatory EHR's Top 10 Players - November 10, 2009

Orlando Florida -- The Ambulatory EHR market is far more challenging than the hospital-based EHR market, said Brian McCarthy, CEO of Sencilo HealthIT Solutions. “You have all these decentralized independent businesses... I think what’s challenging is that these providers are trying to run their small businesses.”

Sencilo's core business is with practices of ten providers or fewer. he believes that’s where the bulk of ambulatory EHR implementations are left to go. “They’re feeling pressure to adopt these EHRs and it’s a huge workflow change. My hope is that the time they have to implement is adequate enough that they can move towards a higher level of use,” he said.

Between 17 percent and 20 percent of practices currently use an EHR, with average installation costs of roughly $40,000 to $60,000. “People are starting off with e-prescribing and reviewing lab results,” said McCarthy. “Moving to the next level, where you’re looking to capture-specific measures and clinical decision support, it’s a huge difference.”

“I think where the vendors can help is really in developing their software further to achieve some of the guidelines here but also being able to create implementation cycles that are reasonable.”


THREE TRENDS TO WATCH

1. ADOPTION: A “higher level” of ambulatory EHR adoption as more physicians move beyond the installation phase.
2. PARTNERSHIPS: Addressing interoperability challenges may mean an increase in the number of vendor partnerships in the ambulatory EHR space. “The vendors are at a point where they need partners to help them implement,” McCarthy said.
3. INCENTIVES: Adoption incentives for EHRs under ARRA will certainly have a positive effect on the number of clinics using electronic records, but financial challenges persist. “It’s hard because they’re retrospective incentives, and you have to have been implemented for a time period to get incentives, which implies you need a loan or grant, and in this economy it’s a hard thing to do,” notes McCarthy.

Why Physicians are worried about with EHRs disrupting work - November 10, 2009

Orlando Florida -- Physicians have limited knowledge of the American Recovery and Reinvestment provisions, according to a recent online survey of 1,000 physicians, and they are still reluctant to adopt information technology.

UnitedHealth Group, conducted the survey, While financial incentives and penalties drive physicians to adopt health IT, the survey showed, they continue to be worried about upfront cost and workflow disruption.

The survey showed more than half, or 58 percent, of the respondents have little or no familiarity with ARRA, with 42 percent indicating some familiarity.

Eighty-two percent cite investment reimbursement as a motivator for implementing an electronic health record system, with 77 percent citing penalties as a motivation to implement.

Not surprisingly, cost and workflow disruption were listed as the two greatest risks for EHR deployment, with 82 percent citing cost and 74 percent citing workflow disruption.

The shift to adopt health IT is happening - thanks to the stimulus funds and CMS incentives - albeit slowly, said Brian McCarthy CEO of Sencilo HealthIT Solutions and Allscripts partner. "There is a growing awareness that the reason to adopt makes sense," he said.

The industry needs to educate the independent physicians who have not had a compelling business case to adopt in the past, McCarthy said. The legacy vendor community has largely focused on the multi-specialty, complex healthcare systems with deep pockets, he said.

With ARRA and a larger market of small physician offices, vendors have an incentive to develop innovative next-generation electronic health records, he said. Hosted EHRs with monthly subscription fees are providing much-needed new business and technology models for independent physicians.

The way physicians are educated on health IT and EHRs also needs to change, McCarthy said, adding, "This is an opportunity to start talking about capabilities in the physician's office."

Technology should be seen as an enabler that brings new capabilities and efficiencies to a physician's office, he said. Physicians will be more receptive to health IT if the conversation is around better care and more efficiency of the whole system.
For instance, physicians would value an EHR with its ability to identify their diabetic patients who have not had a hemoglobin A1c screening test in the past year.

The primary care physician's world is changing rapidly, McCarthy said. Fee and reimbursement schedules are changing, models of care such as the patient-centered medical home are being introduced, reporting quality meausures is required for participation in pay-for-performance programs. The small physician offices are overwhelmed with administrative complexity, clinical protocols and revenue cycle management issues, he said.

Legacy vendors have under-invested in the small group practices in the past. "You have to look at the entire physician's office," McCarthy said. Vendors and independent physicians need to establish the foundational capability in the office and the workflow around that in order to reap the benefits of health IT implementation.

Florida in beta-testing stage of PHR for Medicaid recipients - November 10, 2009

Orlando Florida -- Officials from Florida's Agency for Health Care Administration tested a free personal health record for the state's 2.6 million Medicaid recipients on Wednesday.

The agency previewed the first phase of Florida's Internet-based PHR called My Florida Health eBook and My Florida eBaby Book. In the first phase of the pilot Medicaid recipients will be able to track information about their healthcare visits, procedures, medications, immunizations, allergies and more, says Brian McCarthy from Sencilo HealthIT Solutions and well known speaker on the subject of Healthcare IT.

According to state officials, Florida is the first state to offer a free PHR for Medicaid recipients. The PHR is being funded by a partnership between Florida's Agency for Health Care Administration, Availity, LLC, a Jacksonville, Fla.-based health information network provider, HealthTrio, LLC, an e-health applications and business systems provider in Centennial, Colo., and Healthwise, a nonprofit organization based in Boise, Idaho.

"With Healthwise content, the Agency for Health Care Administration can offer Florida's Medicaid recipients easy access to health information that can help them take an active role in their own health and in their family's health," said Donald W. Kemper, Healthwise chairman and CEO. "The right information at the right time can help a person with diabetes better manage their blood sugar or help a new mom know what to do when her child is sick."

Florida's Agency for Health Care Administration will be meeting with St. Joseph's Children's Hospital in Tampa, Sacred Heart Children's Hospital in Pensacola, Joe DiMaggio Children's Hospital in Hollywood and Arnold Palmer Hospital for Children in Orlando to discuss a pilot of My Florida eBaby Book with Medicaid recipients prior to the official launch.

Officials said that within the next 30 days, Florida's Medicaid recipients will be able to access their Internet-based PHR at www.FloridaHealthFinder.gov using their Medicaid ID number.

"Thanks to this extraordinary partnership with private sector leaders we were able to develop free personal health records for every Medicaid recipient in the state of Florida," said Secretary Holly Benson. "We are the first state in the country to do this, and we believe that this is a tremendous opportunity to empower our recipients with their health care information and to give them the tools they want and need to improve their health."

In the second phase of My Florida Health eBook and My Florida eBaby Book, the Florida's Agency for Health Care Administration will populate each Medicaid recipient's PHR with claims data from services paid for by Florida Medicaid. The recipients will also be able to view their PHR in Spanish.
